Requesting a new smear taker code

Requesting a new cervical sample taker code can be made via the CSW Information Team or your local Cervical Screening Administration Department (CSAD). Trainees are encouraged to register well in advance of beginning to take cervical samples.

You will be sent an information form asking for the following details:

  • name
  • date of birth – not essential
  • whether a doctor / nurse / accredited or trainee colposcopist
  • GMC / NMC code
  • location(s) where cervical samples taken – identifying the main location for correspondence purposes
  • date last received cervical sampling training and type of training (conventional / LBC and initial / update training)

Once these details have been logged, a code card and confirmation form will be returned to you. Date and sign the form, confirming your registered details and receipt of the card.

CSW always ask GPs and individual cervical sample takers to inform CSW when cervical sample takers no longer require their code or if they have received a second code in error.

Asking for a forgotten smear taker code

Please contact your local CSAD or the CSW Information Team, providing the following information:

  • name
  • date of birth – not essential
  • whether a doctor / nurse / accredited or trainee colposcopist
  • location(s) where cervical samples taken

A code can be given over the phone as it is not confidential information.

Notifying a change to smear taker details

Please contact your local CSAD or the CSW Information Team. It is important we are notified of a change to:

  • name details
  • training updates
  • main location, which is used for all correspondence
  • retirement or will no longer be working in Wales

You will be sent a change of details form.

Identifying a smear taker code on a report

Please contact the CSW Information Team who will confirm details on the report before releasing the name of the cervical sample taker.

CSW will always check the location that the caller is ringing from. If the cervical sample taker code that they are requesting identification of, is registered at another address the name will not be given. CSW will only give out the information to a recognised individual at a recognised location.

Information or statistics requests

Information or statistics requests must be made in writing and referred to the Nurse Co-ordinator or the Programme Co-ordinator of your local CSAD.
